Position Summary: As a Home Health Physical Therapist, you will play a crucial role in helping patients regain their independence and improve their overall well-being. You will assess patients' physical conditions, develop personalized treatment plans, and provide therapeutic interventions to address various musculoskeletal, neurological, and cardiopulmonary conditions.

Responsibilities:

• Conduct comprehensive physical evaluations to assess patients' functional abilities and identify treatment goals.
• Develop and implement individualized treatment plans, including therapeutic exercises, modalities, and activities of daily living (ADLs) training.
• Monitor patients' progress and adjust treatment plans as needed.
• Educate patients and their families about self-care techniques and home exercise programs.
• Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated care.
• Maintain accurate and up-to-date patient records.
• Adhere to all company policies and procedures, as well as state and federal regulations.

Qualifications:

• Puerto Rico physical therapy license
• Minimum of 2 years of experience in home health or outpatient physical therapy preferred
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Passion for helping patients achieve their rehabilitation goals
