Full-time | Monday - Friday | 8:00am - 5:00pm
Hourly pay: $31.40 - $37.44 plus mileage reimbursement or company car program
Generous PTO, holiday pay, 401(k) match, medical, dental, vision insurance, HSA with company contribution
CEU reimbursement up to $400 annually and 2 days of CEU time off
Scrubs allowance
Key Responsibilities:
Provide home care physical therapy services under the direction of a licensed Physical Therapist
Participate in interdisciplinary patient assessments and care planning
Complete timely, clear and accurate documentation of treatment and patient progress
Educate patients, families, and caregivers to promote progress toward goals
Must Have + Certifications:
Active Physical Therapist license in Minnesota in good standing
Two years of PT experience preferred
Valid driver’s license, reliable vehicle, and proof of auto insurance
CPR/BLS certification
