Are you an RN with MDS experience who knows how to drive both clinical outcomes and reimbursement?

A respected senior living provider is seeking a strong MDS Coordinator (RN) to lead assessments, care planning, and PDPM optimization—playing a critical role in both resident care and facility performance.

Responsibilities:

• Complete and manage all MDS assessments
• Ensure CMS compliance and documentation accuracy
• Drive PDPM reimbursement optimization
• Develop and update resident care plans
• Conduct audits and support survey readiness
• Collaborate with nursing and interdisciplinary teams

Qualifications:

• Active RN license (MN)
• MDS experience in LTC/SNF preferred
• Knowledge of PDPM & CMS regulations
• EMR experience (PointClickCare a plus)
