Responsibilities:

• Conduct comprehensive physical assessments and develop individualized treatment plans
• Provide physical therapy interventions, including exercises, modalities, and gait training
• Educate patients and their families on self-care techniques and home exercise programs
• Collaborate with other healthcare professionals to ensure optimal patient outcomes
• Maintain accurate and up-to-date medical records

Qualifications:

• Doctor of Physical Therapy (DPT) degree
• Current physical therapy license in the state of California
• Experience in home health or outpatient physical therapy preferred
• Excellent communication and interpersonal skills
• Strong organizational and time management abilities
• Passion for helping patients achieve their rehabilitation goals
