Our team is seeking a dedicated and experienced MDS Coordinator to join our long-term care community. The ideal candidate will be a Registered Nurse (RN) with a strong background in clinical assessment and care planning. The MDS Coordinator will be responsible for the accurate and timely completion of the Minimum Data Set (MDS) assessments and ensuring compliance with state and federal regulations.
Key Responsibilities:
- Coordinate and oversee the completion of the MDS assessments for all residents in accordance with state and federal regulations.
- Ensure the accuracy and timeliness of all MDS submissions to CMS.
- Collaborate with the interdisciplinary team to develop and update comprehensive care plans based on the MDS assessments.
- Monitor and track the submission of MDS assessments to ensure compliance with required timelines.
- Conduct regular audits of MDS documentation to ensure accuracy and completeness.
- Provide education and training to nursing staff on MDS processes and documentation requirements.
- Participate in quality improvement initiatives and regulatory compliance activities.
- Communicate effectively with residents, families, and healthcare team members to ensure high-quality resident care.
Qualifications:
- Current and valid RN license in Minnesota.
- Minimum of 2 years of experience in long-term care or a related healthcare setting.
- Previous experience as an MDS Coordinator or in a similar role preferred.
- Excellent organizational and time management skills.
- Proficient in electronic health records (EHR) systems and MDS software.
- Strong attention to detail and accuracy in documentation.
- Effective communication and interpersonal skills.
- Ability to work independently and as part of a team.
