Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our long-term care team. The LPN will provide high-quality nursing care to residents, ensuring their health, safety, and comfort while promoting independence and dignity. This role involves direct patient care, medication administration, and collaboration with healthcare professionals to enhance resident well-being.
Key Responsibilities:
- Provide direct nursing care to residents in accordance with their individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor residents’ health status, including vital signs and changes in condition, and report concerns to the RN or physician.
- Assist with activities of daily living (ADLs), including bathing, dressing, and mobility support.
- Maintain accurate and timely documentation of residents’ conditions, treatments, and progress in medical records.
- Collaborate with interdisciplinary teams, including RNs, CNAs, therapists, and physicians, to ensure holistic care.
- Provide emotional support and education to residents and their families regarding health conditions and care plans.
- Ensure compliance with facility policies, state regulations, and infection control protocols.
- Respond to emergencies promptly and effectively.
- Supervise and mentor Certified Nursing Assistants (CNAs) as needed.
Qualifications & Requirements:
- Current and valid LPN license in the state of employment.
- CPR/BLS certification (or ability to obtain upon hire).
- Experience in long-term care, geriatrics, or skilled nursing facilities preferred.
- Strong knowledge of nursing best practices, medications, and chronic disease management.
- Excellent communication and interpersonal skills to interact with residents, families, and healthcare teams.
- Ability to work in a fast-paced, team-oriented environment.
- Proficiency in electronic health records (EHR) systems preferred.
Schedule & Work Environment:
- Shifts may include days, evenings, nights, weekends, and holidays.
- Physical demands include lifting, standing, bending, and assisting residents with mobility.
- Work environment includes direct patient contact in a skilled nursing or assisted living setting.
