Responsibilities:
- Provide comprehensive primary healthcare services to individuals of all ages in the community, promoting preventive care and managing chronic conditions.
- Conduct health assessments, perform physical examinations, and diagnose and treat common illnesses and injuries.
- Collaborate with a dedicated team of healthcare professionals, including nurses, physicians, and social workers, to deliver coordinated and holistic care.
- Develop and implement community health programs and initiatives to address prevalent health issues, such as health education workshops, vaccination drives, and wellness campaigns.
- Conduct home visits and outreach programs to reach underserved populations, ensuring equitable access to healthcare services.
- Build strong relationships with community organizations, schools, and local authorities to advocate for health promotion and disease prevention.
- Participate in community events and health fairs to provide screenings, health counseling, and promote the clinic's services.
Qualifications:
- Current Nurse Practitioner certification and a valid nursing license in the state.
- Experience working in a community health setting or a strong passion for community-based healthcare.
- Excellent clinical skills with a focus on preventive care, health promotion, and chronic disease management.
- Strong communication and interpersonal skills to engage with diverse populations and build rapport with patients.
- Ability to work autonomously as well as collaboratively within an interdisciplinary team.
- Flexibility and adaptability to thrive in a dynamic and ever-changing community healthcare environment.
- Bilingual or multilingual skills are a plus, as they will enhance communication with diverse populations.
