You'll play a vital role in supporting patients and their families through effective coordination and administrative tasks.

Responsibilities:

• Collaborate in coordinating patient care plans under supervision.
• Assist in scheduling home visits and ensuring timely care services.
• Communicate effectively with healthcare professionals, patients, and families.
• Maintain accurate records of patient care plans and modifications.
• Support patient and family education on available resources and services.

Additional Qualifications:

• A team player, problem solver, and effective communicator.
• Strong social skills and the ability to build relationships.
• Willingness to travel within the designated territory to various facilities.
• Proficient in Microsoft Suite and basic computer knowledge.
• Eagerness to learn, grow, and take on new challenges.

Daily Tasks Include:

• Engaging with office managers, physicians, and patients.
• Documenting and promoting the company's services.
• Traveling to different facilities to represent services.
• Generating referrals and aiding facilities with their needs.
• Identifying patients requiring additional care and obtaining approvals.
