RN Case Manager - Home Care in Minneosta

Key Responsibilities

  • Conduct comprehensive head-to-toe assessments, care coordination, disease/medication management, skilled wound care, catheter/ostomy care, infection prevention, and home safety evaluations.
  • Visit patients in their homes to evaluate care quality, review clinical notes, and participate in ongoing interdisciplinary assessments.
  • Implement physician-ordered services, educate patients/families on healthcare needs, and complete timely, accurate clinical documentation.
  • Collaborate with the healthcare team to ensure seamless coordination, progress tracking, and discharge planning.

Essential Qualifications

  • Graduate of an accredited nursing program with current, active RN license in Minnesota.
  • Valid driver's license, reliable transportation, auto insurance, and CPR certification.
  • Preferred: 2+ years nursing experience, 1+ year case management, community health, and broad med-surg background.

  • Competitive salary: $89,419–$106,620 annually.

Schedule & Territory

  • Full-time, days (Monday–Friday) with holiday rotation (~2/year) and possible weekend rotation based on staffing.
  • Field-based role serving patients' homes in southeast Twin Cities metro; office in Roseville, MN.
