A respected, locally owned home health agency serving Manitowoc and Sheboygan Counties for over 40 years is seeking a skilled Home Health Physical Therapist to join their clinical team. This organization is known for delivering intelligent, cost-effective, and compassionate care that helps patients maintain independence while receiving high-quality therapy services in their homes.
In this role, you will work directly with patients recovering from illness, injury, or surgery—helping them regain strength, mobility, and confidence while remaining in the place they are most comfortable: their home.
Conduct comprehensive physical therapy evaluations in the home setting
Develop and implement individualized treatment plans to improve mobility, strength, and functional independence
Provide hands-on therapy interventions to support recovery from injury, surgery, or chronic conditions
Educate patients and families on exercise programs, fall prevention, and mobility strategies
Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate care
Monitor patient progress and adjust treatment plans to achieve optimal outcomes
Document patient care accurately in accordance with clinical and regulatory standards
Promote safety and independence while supporting patients’ overall quality of life
Licensed Physical Therapist (PT) in the state of Wisconsin
Home health experience preferred but not required
Strong understanding of rehabilitation, mobility restoration, and functional recovery
Excellent communication and patient education skills
Ability to work independently while managing a flexible caseload
Strong organizational and documentation skills
Compassionate approach with a passion for helping patients remain independent at home
