Hospice Registered Nurse Case Manager (Home Visits)

A respected hospice provider is seeking a compassionate Registered Nurse Case Manager to deliver exceptional home-based hospice care. This is a field position serving patients and families across the greater Cincinnati and South Dayton region, supporting comfort, dignity, and peace through every stage of care.

About the Role

As the RN Case Manager, you’ll take ownership of the patient’s plan of care — providing skilled nursing, symptom management, and emotional support for both patients and families. You’ll be part of a close-knit interdisciplinary team dedicated to holistic, high-quality end-of-life care.

What You’ll Do

  • Conduct comprehensive assessments and establish care plans tailored to each patient’s needs

  • Deliver skilled nursing care, medication management, and symptom relief in the home setting

  • Coordinate closely with physicians, social workers, chaplains, and other care providers

  • Maintain thorough and timely documentation in compliance with hospice standards

  • Educate and support families on care techniques, comfort measures, and the hospice philosophy

What You’ll Need

  • Active RN license in the state of practice

  • At least one year of clinical nursing experience (home health or hospice preferred)

  • Current CPR certification

  • Dependable transportation, valid driver’s license, and auto insurance

  • A heart for patient-centered care and strong communication skills
