A respected hospice organization is expanding its clinical team and seeking an experienced Registered Nurse (RN) Case Manager who is passionate about providing compassionate, high-quality care to patients and families. As a Hospice RN Case Manager, you’ll care for patients in their homes or care facilities, guiding them and their loved ones through the end-of-life journey with dignity and comfort. You’ll work closely with a supportive interdisciplinary team including social workers, chaplains, aides, and volunteers to ensure patients receive the physical, emotional, and spiritual care they deserve.
Manage a caseload of hospice patients, conducting visits in homes, skilled nursing, or assisted living settings
Provide skilled nursing assessments, medication management, and symptom control
Collaborate daily with the hospice care team to create and maintain individualized care plans
Document all care and communications accurately using an electronic medical record system
Prioritize visits and respond to urgent needs with strong time-management and triage skills
Current Registered Nurse (RN) license in good standing within the state of practice
Hospice or home health experience preferred, but not required
Excellent interpersonal communication and patient advocacy skills
Ability to work independently while collaborating closely with a clinical team
Reliable transportation, valid driver’s license, and current auto insurance
