Hospice Registered Nurse Case Manager (Home Visits)

A respected hospice organization is expanding its clinical team and seeking an experienced Registered Nurse (RN) Case Manager who is passionate about providing compassionate, high-quality care to patients and families. The Opportunity

As a Hospice RN Case Manager, you’ll care for patients in their homes or care facilities, guiding them and their loved ones through the end-of-life journey with dignity and comfort. You’ll work closely with a supportive interdisciplinary team including social workers, chaplains, aides, and volunteers to ensure patients receive the physical, emotional, and spiritual care they deserve.

What You’ll Do

  • Manage a caseload of hospice patients, conducting visits in homes, skilled nursing, or assisted living settings

  • Provide skilled nursing assessments, medication management, and symptom control

  • Collaborate daily with the hospice care team to create and maintain individualized care plans

  • Document all care and communications accurately using an electronic medical record system

  • Prioritize visits and respond to urgent needs with strong time-management and triage skills

Qualifications

  • Current Registered Nurse (RN) license in good standing within the state of practice

  • Hospice or home health experience preferred, but not required

  • Excellent interpersonal communication and patient advocacy skills

  • Ability to work independently while collaborating closely with a clinical team

  • Reliable transportation, valid driver’s license, and current auto insurance
