Registered Nurse – Home Health (Full Time)

A respected home health provider is seeking an experienced and compassionate Registered Nurse (RN) to join its growing clinical team. This is a full-time field position serving patients across the greater Cincinnati area, delivering high-quality, personalized care right in the comfort of their homes.

About the Role

As a Home Health RN, you’ll work closely with physicians and an interdisciplinary care team to ensure every patient receives exceptional, evidence-based care. You’ll have the independence of home visits combined with the full support of a collaborative leadership team that understands the realities of clinical work.

Key Responsibilities

  • Coordinate with physicians to manage medications and update plans of care

  • Supervise LPNs, aides, and therapy staff to maintain consistent, high-quality patient outcomes

  • Perform comprehensive assessments, OASIS documentation, recertifications, and discharges using an EMR system

  • Help reduce hospital readmissions through proactive case management and early intervention

  • Participate in team meetings, continuing education, and orientation programs

Qualifications

  • Active Registered Nurse (RN) license in good standing within the state of Ohio

  • Current CPR certification

  • Reliable transportation, valid driver’s license, and proof of auto insurance

  • Working knowledge of Medicare/Medicaid regulations for Home Health

  • Strong communication, organization, and patient-advocacy skills
