Job Description

We are currently seeking a skilled and experienced Registered Nurse (RN) who is ready to hit the ground running and make a real difference in people’s lives—someone who leads with compassion, shows up with heart, and is ready to bring quality care directly into the homes of those who need it most.

Position Summary: As a Home Health RN, you will play a vital role in developing, implementing, and evaluating individualized patient care plans. You'll work closely with physicians, therapists, and other healthcare professionals to ensure our patients receive the highest quality of care.

Key Responsibilities:

  • Conduct in-home patient assessments and develop personalized care plans

  • Administer medications, wound care, and other skilled nursing interventions

  • Monitor patient health and update care plans accordingly

  • Educate patients and families on treatment, medications, and self-care techniques

  • Maintain accurate and timely documentation in accordance with agency and regulatory standards

  • Communicate effectively with physicians, case managers, and fellow care team members

Qualifications:

  • Current RN license in Oklahoma (required)

  • Minimum of 1 year of home health nursing experience (required)

  • Familiarity with OASIS documentation (preferred)

  • CPR Certification (required)

  • Valid driver’s license, reliable transportation, and auto insurance

  • Excellent communication, clinical, and time-management skills

  • A warm, patient-first approach and the ability to work independently
