Job Description

We are seeking a skilled and compassionate Home Health Care Physical Therapist to join our team as a leading provider of home health care services dedicated to delivering high-quality care to patients in the comfort of their own homes. Job Description: The ideal candidate will provide exceptional therapy services to patients in their homes, developing personalized treatment plans to help them recover, regain mobility, and enhance their overall quality of life. This role requires strong clinical skills, a passion for patient care, and the ability to work independently in a home-based setting.

Key Responsibilities:

  • Conduct thorough physical therapy evaluations for patients in their homes.
  • Develop and implement individualized treatment plans based on patient assessments and goals.
  • Provide hands-on therapy and education to patients and their families on exercise programs, safety techniques, and mobility aids.
  • Monitor patient progress, adjust treatment plans as needed, and document patient care and progress in compliance with regulatory standards.
  • Collaborate with interdisciplinary team members, including physicians, nurses, and occupational therapists, to coordinate patient care.
  • Educate patients and families on home exercise programs, injury prevention, and lifestyle modifications to support recovery and independence.

Qualifications:

  • Licensed Physical Therapist (PT) in California.
  • Geriatric experience preferred but not required.
  • Strong clinical skills with a focus on patient-centered care.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and manage time effectively.
  • Valid driver’s license, reliable transportation, and current auto insurance.
