We are seeking a compassionate and dedicated Physical Therapist to join our team at our Long-Term Care Community in the Minneapolis area. As a Physical Therapist, you will play a vital role in enhancing the quality of life for our elderly residents by providing expert evaluation, treatment, and rehabilitation services. Your primary focus will be to promote mobility, independence, and overall well-being in a supportive and empathetic environment.
Key Responsibilities:
Assessment and Evaluation: Conduct thorough evaluations of residents’ physical conditions, functional abilities, and mobility needs. Develop individualized treatment plans based on assessments.
Treatment and Rehabilitation: Implement evidence-based therapeutic interventions to address residents’ specific needs, including exercises, manual therapy, and assistive devices. Monitor progress and adjust treatment plans as necessary.
Care Coordination: Collaborate with other healthcare professionals, including nurses, physicians, and occupational therapists, to ensure comprehensive and coordinated care. Communicate effectively with residents and their families about treatment goals and progress.
Education and Support: Provide education to residents, families, and staff on techniques to enhance mobility, prevent falls, and manage chronic conditions. Empower residents to take an active role in their rehabilitation process.
Documentation: Maintain accurate and detailed records of assessments, treatment plans, progress notes, and outcomes in accordance with facility policies and regulatory requirements.
Compliance and Safety: Adhere to all safety protocols, infection control procedures, and regulatory standards to ensure a safe and supportive environment for residents.
Qualifications:
Education: Degree in Physical Therapy from an accredited program.
Licensure: Valid Physical Therapist license in the state of Minnesota.
Experience: Previous experience working with elderly populations, preferably in a long-term care or similar setting. Demonstrated expertise in geriatric rehabilitation preferred.
Skills: Strong clinical skills, including assessment and treatment planning. Excellent communication and interpersonal skills, with the ability to build rapport and demonstrate empathy with residents and their families.
