Position Summary
A respected human services organization is seeking a Registered Nurse to join their clinical team to oversee and coordinate health care services for individuals served within a residential and day program setting. The Nurse will operate under the guidelines of the New York State Nurse Practice Act, NYS Office of Children and Family Services (OCFS), and agency policies.
This is a confidential search on behalf of a well-established, trauma-informed program supporting individuals with diverse health needs.
Primary Duties & Essential Functions
Maintain compliance with 29I licensure requirements and NYSED guidelines
Complete thorough nursing assessments for new admissions and ongoing care
Accurately complete and maintain health records including Emergency Room Info, Medication Administration Records (MARs), and Health Files
Develop individualized Nursing Care Plans based on comprehensive assessments
Coordinate lab work and healthcare appointments; prepare consult notes and follow up on provider recommendations
Collaborate with clinicians, case coordinators, and direct care staff to monitor and support individual health and personal care needs
Manage medication supplies, conduct medication administration and counts, and ensure accurate documentation and medication safety
Participate in the Nursing On-Call system and respond to healthcare needs as they arise
Assess and support individuals post-hospital or psychiatric admissions
Maintain immunization records and ensure compliance with state reporting requirements
Assist with adaptive equipment procurement and medical follow-ups as needed
Required Education, Knowledge & Skills
Current New York State Registered Nurse license and registration
Experience providing health services in school, residential, or community settings preferred
Strong clinical assessment and documentation skills
Ability to collaborate effectively with multidisciplinary teams
Valid NYS driver’s license
Abilities & Working Conditions
Ability to work a full 40-hour week with occasional flexibility
Physical capability to lift up to 25 lbs and stand or move for extended periods
