Licensed Practical Nurse (LPN)

We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join a skilled nursing and rehabilitation team. As an LPN, you will play an important role in supporting residents' health and well-being while collaborating with an interdisciplinary team in a supportive, patient-centered environment.

Compensation

$33.00 - $43.00 per hour, depending on experience, qualifications, and skill set.

Key Responsibilities

  • Administer medications and prescribed treatments according to physician orders and care plans
  • Monitor residents' health status and perform ongoing assessments
  • Document resident care and maintain accurate medical records
  • Assist with developing, coordinating, and updating individualized care plans
  • Support and guide Certified Nursing Assistants (CNAs) in daily resident care activities
  • Serve as a resource for assigned units and assist with supervisory duties as needed
  • Collaborate with healthcare providers, residents, and family members to ensure quality care outcomes

Qualifications

Required

  • Current Licensed Practical Nurse (LPN) license in good standing
  • Current CPR and First Aid certification
  • Two-step TB clearance
  • Successful completion of background screening, fingerprinting, and drug testing requirements
  • Strong verbal and written English communication skills

  • Excellent clinical judgment and critical thinking abilities
  • Experience assessing resident care needs and responding appropriately
  • Ability to manage resident behaviors with professionalism and compassion
  • Leadership and supervisory skills to support nursing assistants and maintain departmental standards
  • Strong interpersonal skills and commitment to maintaining resident confidentiality
  • Ability to work effectively with residents, families, healthcare providers, and multidisciplinary teams
