Licensed Practical Nurse (LPN) – Assisted Living / Memory Care

Location: Greater Indianapolis Area, Indiana
Type: Full-Time, On-Site
Compensation: $54,000 – $68,000 annually (depending on experience and shift) + benefits

About the Opportunity

We’re representing a respected assisted living and memory care community seeking a dedicated Licensed Practical Nurse (LPN) to join their clinical team.
This role offers a meaningful opportunity to provide hands-on resident care while working within a supportive, mission-driven environment that values teamwork and compassion.

Key Responsibilities

• Administer medications and treatments per physician orders.
• Monitor residents’ health status and communicate changes promptly to the Wellness Director or RN.
• Supervise Resident Assistants (RAs) and Qualified Medication Aides (QMAs).
• Maintain accurate and timely documentation in resident records.
• Support care plan updates and compliance with state and facility standards.
• Assist with admissions, discharges, and communication with families.

Qualifications

• Active LPN license in the state of Indiana.
• Current CPR and First Aid certification.
• Previous experience in long-term care, assisted living, or memory care preferred.
• Strong clinical judgment, communication, and leadership skills.
