Physical Therapist, Home Health

One of our top clients, a respected home health agency in Knoxville, TN, is seeking a skilled and compassionate Physical Therapist to join its field team. The ideal candidate enjoys one-on-one patient care in the home and wants autonomy, flexibility, and the chance to make a real difference in patients' recovery. This is a full-time role.

Responsibilities:

• Perform patient evaluations and develop individualized plans of care
• Provide skilled physical therapy visits to patients in their homes
• Supervise and collaborate with Physical Therapist Assistants
• Educate patients and families on home exercise programs, safety, and fall prevention
• Complete OASIS assessments and visit documentation accurately and on time
• Coordinate care with nurses, physicians, and the interdisciplinary team

Qualifications:

• Active Physical Therapist license in Tennessee
• Doctor of Physical Therapy (DPT) or master's in Physical Therapy from an accredited program
• Home health experience preferred; strong PTs from other settings are welcome to apply
• Current CPR/BLS certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to work independently and as part of a team.
