Position Overview
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our dedicated care team on the 2nd or 3rd shift. The ideal candidate will deliver quality nursing care under the supervision of a registered nurse or physician, ensuring exceptional outcomes and comfort for our residents.

Key Responsibilities

• Administer prescribed medications safely and accurately
• Record and monitor resident vital signs and health status
• Support resident assessments, documentation, and care planning
• Collaborate closely with physicians, RNs, and CNAs to meet resident needs
• Provide both physical and emotional support to residents and their families

Qualifications

• Active LPN license in the State of Wisconsin
• Strong communication, organization, and teamwork skills
• Comfortable working in a fast-paced, collaborative environment
• Basic computer proficiency for documentation and charting
• Knowledge of medical terminology and nursing procedures
