What you’ll do

  • Provide leadership and direction to the hospice clinical team to ensure safe, effective, and efficient delivery of hospice services.
  • Plan, assign, and monitor the daily work of hospice staff to ensure optimal caseload balance, timely visits, and excellent patient and family experience.
  • Hire, supervise, evaluate, coach, and develop professional and paraprofessional team members, fostering a positive, accountable, and collaborative culture.
  • Oversee staff scheduling to maximize productivity while supporting work–life balance and continuity of care.
  • Lead Interdisciplinary Group (IDG) meetings, ensuring comprehensive, patient-centered care planning across nursing, social work, spiritual care, aides, and other disciplines.
  • Actively participate in facility meetings, team huddles, and cross-functional initiatives as requested.
  • Manage a limited hospice caseload when needed, maintaining strong clinical competence and visibility in the field.
  • Partner with leadership to create and implement recruitment, orientation, and onboarding strategies that attract and retain high-quality hospice talent.
  • Provide ongoing education, in-services, and coaching to hospice staff to ensure compliance with agency policies, payer requirements, and all applicable federal, state, and accreditation standards.
  • Lead communication and implementation of Quality Assurance and Performance Improvement (QAPI) initiatives, including sharing updates, tracking outcomes, and driving performance.
  • Represent the hospice program in the community by providing education, information, and support to referral sources, facilities, and other external partners.
  • Collaborate with marketing and outreach teams to support business growth and strengthen referral relationships.

What you bring

  • Graduate of an accredited school of nursing.
  • Current, active Registered Nurse (RN) license in the State of Minnesota, in good standing.
  • Prefer at least two (2) years of experience as a hospice RN.
  • Prefer at least one (1) year of supervisory or leadership experience (e.g., charge RN, clinical supervisor, manager, or director), ideally in hospice or home health.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Strong clinical assessment skills and deep commitment to high-quality, patient- and family-centered end-of-life care.
  • Proven ability to lead, mentor, and motivate interdisciplinary teams.
  • Excellent communication, organization, and problem-solving skills.
  • Comfort with balancing clinical excellence, regulatory compliance, and operational efficiency.
