Salary: $90,000 - $120,000 (DOE) + Bonus Opportunities

About the Role:

A leading long-term care and rehabilitation provider is seeking a licensed Occupational Therapist (OT) to join our team in the San Francisco Bay Area. The OT will play a key role in helping residents regain independence and improve their quality of life through personalized therapy and rehabilitation programs in skilled nursing facilities (SNFs) and assisted living communities.

Key Responsibilities:

  • Evaluate and develop individualized treatment plans for residents in skilled nursing and long-term care settings.
  • Provide rehabilitative therapy to improve patients’ mobility, strength, and daily function.
  • Collaborate with nurses, physical therapists, and speech therapists to implement holistic care plans.
  • Educate patients and caregivers on adaptive techniques and equipment for home and facility use.
  • Ensure compliance with Medicare, state, and federal regulations.
  • Document patient progress and maintain accurate therapy records.

Qualifications:

  • Valid CA Occupational Therapist (OT) license (or eligibility to obtain).
  • 1+ years of experience in long-term care, skilled nursing, or rehabilitation preferred.
  • Strong knowledge of Medicare guidelines, therapy documentation, and treatment planning.
  • Passion for working with aging populations and individuals with disabilities.
  • Excellent communication and teamwork skills.
