Position Overview

We are seeking a Clinical Manager (RN) to oversee and support a team of nurses in providing exceptional case management and care coordination services. This role requires a licensed RN with strong leadership skills and a passion for improving patient outcomes.

Key Responsibilities

  • Lead, train, and support a team of Registered Nurse Case Managers.
  • Ensure high-quality care coordination for patients with complex medical conditions.
  • Develop and implement care plans to enhance patient outcomes.
  • Collaborate with healthcare providers, patients, and families to ensure seamless communication and care transitions.
  • Monitor compliance with healthcare regulations and company protocols.
  • Provide clinical guidance to the nursing team.
  • Conduct performance evaluations and support professional development for nursing staff.

Qualifications

  • Active RN License (Required).
  • Previous leadership or management experience in case management or healthcare (Preferred).
  • Experience in care coordination, case management, or community-based nursing (Preferred).
  • Strong communication, organization, and leadership skills.
  • Ability to work both independently and collaboratively within a healthcare team.
