MDS Coordinator (RN) – Long Term Care

Key Responsibilities:

  • Oversee the Resident Assessment Instrument (RAI) process, ensuring timely and comprehensive assessments.
  • Develop individualized care plans that meet each resident’s unique needs.
  • Provide staff education on the RAI process and ensure compliance with regulations.
  • Conduct intake assessments to evaluate care needs and financial implications.
  • Participate in Quality Assurance (QA) activities to maintain high standards of care.
  • Support electronic medical records (EMR) development and utilization within the facility.

What You Need to Succeed:

  • Valid RN license in good standing.
  • MDS experience in a Long-Term Care setting.
  • MDS certification (preferred).
  • Nursing degree (preferred but not required).
