Job Description

We are seeking a dedicated and skilled Registered Nurse to join our team as a Nurse Educator. This role is integral to providing exceptional care and education to our participants, ensuring their health needs are met and that they are prepared for increased independence.

Key Responsibilities:

  1. Interdisciplinary Team Collaboration:

    • Act as a key member of each participant’s interdisciplinary team.
    • Complete or obtain necessary medical assessments as required by the team and accreditation standards.
  2. Participant Health Management:

    • Ensure and document that each participant’s health needs are addressed as per their Individual Service Plan.
    • Assist staff in making informed decisions about participants’ medical needs and referrals to physicians.
    • Accompany participants to medical appointments for complicated issues, review results, and provide staff with appropriate instructions.
    • Complete 24-hour nursing assessments for participants.
    • Ensure medications and medical care are administered as prescribed by physicians.
    • Maintain and update medication and immunization histories for each participant.
    • Provide health care and sexuality education to participants to promote independence.
    • Review and approve objectives for teaching Self-Administration of Medications prior to implementation.
  3. Medication Management:

    • Collaborate with a consultant pharmacist to meet participants’ medication needs.
    • Place and verify medication orders.
    • Ensure all required medications are available for administration and that side effect sheets are present.
    • Review and prepare the Medication Administration Record (MAR) for the upcoming month.
    • Check MARs, medication bubble packs, and medication bottles for accuracy.
    • Oversee the pharmacy’s services to the agency.
    • Ensure availability of over-the-counter medications and first aid supplies; inspect medication carts weekly.

Qualifications:

  • Valid Registered Nurse (RN) license.
  • Experience in a similar role is preferred.
  • Strong knowledge of medical and nursing practices.
  • Excellent organizational and communication skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Commitment to participant education and health management.
